Arteriyovenöz fistül operasyonlarında supraklaviküler brakiyal pleksus bloğunun doku oksijen satürasyonu üzerine etkisi

Özet
Amaç: Çalışmamızda son dönem böbrek yetmezliği nedeniyle arteriyovenöz (AV) fistül açılacak hastalarda supraklaviküler brakiyal pleksus bloğunun lokal infiltratif anestezi ile karşılaştırılmak suretiyle doku oksijenizasyonu üzerine olan etkisini araştırılması amaçlanmıştır. Metod: Arteriyovenöz fistül açılan 60 hasta ile yapılan çalışmada Grup L hastalarına 15 ml %5 bupivakain ile lokal infiltratif anestezi uygulanırken, Grup B hastalarına ultrason eşliğinde periferik sinir stimülatörü kullanılarak 20ml %5 bupivakain ve 10 ml %2 lidokain ile supraklaviküler brakial pleksus bloğu uygulandı. Sedasyon düzeyi Ramsay Sedasyon Skoru ile değerlendirilirken Grup B hastalarının duyusal bloğu "three point" skala "Pinprick testi" ve motor bloğu Modifiye Bromage Skalası ile aralıklarla değerlendirildi. Supraklaviküler brakiyal pleksus bloğu uygulamasını takiben Modifiye Bromage Skalasının grade III, "three point" skalasının grade II olduğu süreler ve 5 dakika aralıklarla her iki elden alınan NIRS değerleri kaydedildi. İntraoperatif cerrah memnuniyeti ve hasta memnuniyeti Likert memnuniyet değerlendirme ölçeği ile değerlendirdi. Ameliyattan bir ay sonra hastalar telefonla aranarak, açılan AV fistülün primer açıklığı sorgulandı ve gruplar karşılaştırıldı. Bulgular: Grupların demografik verileri incelendiğinde hastaların vücut kitle indeksi (VKİ), primer renal hastalıkları ve komorbiditeleri benzer bulundu. Ortalama arteriyel basınçları ve kalp tepe atımlarında gruplararası fark yoktu. AV fistül açılan kolun NIRS değerleri, Grup B'de Grup L'den daha yüksek ölçüldü. Grup B'deki hasta memnuniyeti ve cerrah memnuniyeti Grup L'ye göre daha yüksek bulundu. Primer fistül açıklığı değerlendirilirken Grup B'den bir hastaya ulaşılamadı ve Grup L'den bir hasta ameliyat sonrası 23. gününde eks olduğu için primer fistül açıklıkları değerlendirilemedi. Kalan hastalar karşılaştırıldığında gruplar arasında primer fistül açıklığı açısından istatistiksel olarak anlamlı fark bulunmadı. Sonuç: Son dönem böbrek yetmezliği olan hastalarda AV fistül açılırken lokal infiltratif anesteziyle karşılaştırıldığında brakial pleksus bloğunun doku oksijenizasyonunu artırdığı saptandı. Primer fistül açıklığı açısından brakial pleksus bloğunun lokal infiltratif anesteziye bir üstünlüğü görülmedi. Hastalarımızda brakial pleksus bloğunun hasta ve cerrah memnuniyetini artırdığını saptadık.
Aim: Our study aimed to compare the effect of supraclavicular brachial plexus block on tissue oxygenation in patients undergoing AV fistula operations due to end-stage renal failure; and compared with local infiltrative anesthesia. Materials and Methods: In the study 60 patients undergoing AV fistula opening operations; Group L patients underwent local, infiltrative anesthesia with 15 ml of 5% bupivacaine, while Group B patients underwent a supraclavicular brachial plexus block with 20ml of 5% bupivacaine and 10ml of 2% lidocaine using an ultrasound guided peripheral nerve stimulator. While the sedation level was evaluated with the Ramsay Sedation Score, the sensory block of Group B patients was evaluated at intervals with a three-point scale Pinprick test and motor block with the Modified Bromage Scale. Following the application of supraclavicular brachial plexus block, the durations of the Modified Bromage Scale grade III, three point scale II and NIRS values taken from both hands at 5 minute intervals were recorded. Intraoperative surgeon satisfaction and patient satisfaction were evaluated with a Likert satisfaction assessment scale. Patients were contacted by phone one month after the operation, and the primary patency of the opened AV fistula was questioned. Results: When the demographic data of the groups were examined, the body mass index (BMI), primary renal diseases and comorbidities of the patients were found to be similar. There was no difference in mean arterial pressure and heart rate within groups. The NIRS values of the arm with AV fistula openings were higher in Group B than in Group L. Patient satisfaction and surgeon satisfaction in Group B was higher than in Group L. One patient in Group B could not be reached, and one patient from Group L had died on the 23rd postoperative day. Thus, the primary fistula patency of these patients was not evaluated in these two patients. When the remaining patients were compared, it was found that there was no statistical difference between the groups in terms of primary fistula patency. Conclusion: We found that brachial plexus block increases tissue oxygenation when compared with local infiltrative anesthesia during opening of AV fistulas in patients with end-stage renal failure. There was no superiority of the brachial plexus block to local infiltrative anesthesia in terms of primary fistula patency. We found that brachial plexus block increases patient and surgeon satisfaction, in these patients.
