dc.description.abstractFleplerin mikrosirkülasyonuve sağkalımı, cerrahi başarı ve yara iyileşme sürecinin kritik belirleyicilerindendir; bu nedenle flep üzerindekielektomanyetik alan tedavisininetkilerinin araştırılması klinik açıdan büyük önem taşımaktadır.Bu araştırmadafarklı şiddetlerde çok düşük frekansta elektromanyetik alan tedavisinin sıçanlarda rastgele deri flebi mikrosirkülasyonu ve sağkalımı üzerineetkilerininincelenmesi amaçlanmıştır.Bu deneysel çalışmada, sıçanlarda McFarlane yöntemiyle oluşturulan random cilt flepleri üzerine düşük (0,2 mT) ve yüksek (5 mT) dozda 50 Hz sinusoidal elektromanyetik alan (EMF) uygulanmıştır. Deney, Maltepe Üniversitesi Deney Hayvanları Uygulama ve Araştırma Merkezi'nde, her biri 7 sıçandan oluşan üç grupta yürütülmüştür. EMF uygulamaları, cerrahi sonrası 14 gün boyunca günde iki kez 15 dakikalık seanslarla gerçekleştirilmiştir. Flep sağkalımı makroskopik ölçümlerle, mikrosirkülasyon ise histopatolojik, immünohistokimyasal ve qRT-PCR analizleriyle değerlendirilmiştir. Veriler istatistiksel olarak uygun parametrik veya non-parametrik testlerle analiz edilmiştir.Araştırma grupları toplam flepalanı açısından istatistiksel olarak benzerdir (p=0,140). Düşük doz EMF grubunda sağlıklı flep alanı yüzdesi, hem kontrol hem de yüksek doz EMF grubuna göre anlamlı olarak daha yüksektir (p=0,006 ve p=0,013). Histopatolojik incelemede inflamasyon, kontrolve yüksek doz EMF gruplarında, düşük doz EMF grubuna kıyasla anlamlı derecede artmıştır (p<0,0005). Re-epitelizasyon, vaskülarizasyon ve kollajen birikimi ise düşük doz EMF grubunda belirgin olarak artış göstermiştir (p<0,0003, p<0,0016 ve p<0,0001).Düşük doz EMF, apoptoz genleri (CASP8, BAX, BCL2, CASP9) ekspresyonunu artırırken, yüksek doz EMF düzensiz veya baskılayıcı etkiler göstermiştir. İnflamasyon belirteci TNF-?, düşük dozda 9741 kat artarken, yüksek dozda 11,48 kat artmıştır. Hem düşük hem yüksekEMF, MAPK-1, MAPK-3 ve MAPK-14 gen ekspresyonunu baskılamıştır, bu baskı özellikle yüksek dozda belirgindir.Düşük doz elektromanyetik alan uygulaması yara iyileşmesini desteklerken, yüksek doz EMF'nin inflamasyonu artırarak iyileşmeyi olumsuz etkileyebileceği görülmüştür. EMF, güvenli ve ekonomik bir yöntem olarak potansiyel faydalar sunsa da, etkilerinin tam olarak anlaşılabilmesi için daha kapsamlı çalışmalara ihtiyaç vardır.
dc.description.abstractThe microcirculation and survival of flaps are critical determinants of surgical success and wound healing; therefore, investigating the effects of electromagnetic field therapy on flaps holds significant clinical importance. This study aimed to evaluate the effects of extremely low-frequency electromagnetic field therapy at different intensities on the microcirculation and survival of random skin flaps in rats.In this experimental study, random dorsal skin flaps wereelevatedin rats using the McFarlane method, followed by exposure to low (0.2 mT) and high (5 mT) doses of 50 Hz sinusoidal electromagnetic fields (EMF). The experiment was conducted at the Maltepe University Experimental Animal Research Center with three groups, each consisting of seven rats.EMF exposure was administered twice daily for 15 minutes over a 14-day postoperative period. Flap survival was assessed through macroscopic measurements, while microcirculation was evaluated using histopathological, immunohistochemical, and qRT-PCR analyses. Data were statistically analyzed using appropriate parametric or non-parametric tests.The experimental groups were statistically similar in terms of total flap area (p=0.140). The percentage of healthy flap area in the low- dose EMF group was significantly higher compared to both the control and high-dose EMF groups (p=0.006 and p=0.013, respectively). Histopathological analysis revealed significantly increased inflammation in the control and high-dose EMF groups compared to the low-dose EMF group (p<0.0005). Re-epithelialization, vascularization, and collagen deposition were markedly enhanced in the low-dose EMF group (p<0.0003, p<0.0016, and p<0.0001, respectively). Low-dose EMF significantly upregulated apoptosis-related genes (CASP8, BAX, BCL2, CASP9), whereas high-dose EMF exhibited irregular or suppressive effects. The inflammatory marker TNF-? expression increased 9741-fold in the low-dose group and 11.48-fold in the high-dose group. Both low and high-dose EMF suppressed the expression of MAPK-1, MAPK-3, and MAPK-14 genes, with more pronounced suppression observed at high doses. While low-dose electromagnetic field (EMF) application supports wound healing, high-dose EMF may impair healing by increasing inflammation.
