Alerjik rinitli çocuklarda uykuda solunum bozukluğunun polisomnografi ile değerlendirilmesi

Özet
Giriş ve Amaç: Alerjik rinit tanılı hastalarda nazal obstrüksiyon uyku bozukluklarının temel nedeni olarak kabul edildiğinde, nazal obstrüksiyonun ve dolayısıyla nazal havayolu direncinin tedavi edilmesi ile bu hastalarda uyku kalitesinin artacağı öngörülebilmektedir. Çalışmamızdaki amacımız alerjik rinitli hastalarda polisomnografi, yaşam ve uyku kalitesi anketleri ile uykuda solunum bozukluklarını belirlemek ve medikal tedavi sonrası düzelme olup olmadığını gözlemlemektir. Gereç ve Yöntem: İstanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Çocuk Göğüs Hastalıkları Poliklinig?i'ne Ocak-Kasım 2022 tarihlerinde başvuran uykuda solunum bozukluğu semptomu tariflediği için polisomnografi (PSG) uygulanan 35 persistan orta/ağır ve intermittan orta/ağır Alerjik Rinitli hastanın dosyası retrospektif olarak değerlendirildi. Hastaların sosyodemografik özellikleri (boy, ağırlık, cinsiyet, beden kitle indeksi), Total Nazal Semptom Skorları (TNSS), Çocuklar için Uyku Bozuklukları Ölçeği (ÇUBÖ), PRQLQ (Burun ve Gözlerinde Alerjisi Olan Çocuklar İçin Yaşam Kalitesi Soru Formu), Polisomnografi parametreleri değerlendirildi. Yapılan polisomnografide OUAS (Obstrüktif Uyku Apne Sendromu) tanısı alan ve 8 hafta medikal tedavi uygulanan hastalara kontrol PSG uygulandı. Hastaların tedavi öncesi ve sonrası PSG parametreleri, uyku ve yaşam kalite skorları karşılaştırıldı. Bulgular: Çalışmaya aldığımız 35 alerjik rinit hastasının 15'i kız 20'si erkekti. Persistan ve intermittan orta/ağır AR'li hastaların %42,8 (n=15)'inde OUAS saptadık. OUAS saptanan ve saptanmayan hastaları arasında yaş ve cinsiyet açısından anlamlı bir fark saptanmadı (sırasıyla p=0,693 , p=0250) . OUAS saptanan hastaların 4'ü (%26,6) hafif OUAS, 10'u (%66,6) orta OUAS ve 1'i (%6,6) ağır OUAS tanısı aldı. TNSS ile PRQLQ skoru, uykululuk skoru ve obstrüktif apne sayısı arasında pozitif yönde bir korelasyon saptadık. OUAS saptanan hastaların sekiz haftalık medikal AR tedavisi sonrası yaşam kalitesi, uykululuk ve TNS skorlarında anlamlı iyileşme gördük (her 3 anket için p<0,001). Tedavi sonrası OUAS'lı hastaların kontrol polisomnografi verileri karşılaştırıldığında apne hipopne indeksi (AHİ) ve obstrüktif apne hipopne indeksinde (OAHİ) tedavi sonrası anlamlı düşüş saptandı (p<0.001). Tedavi başlangıcında ağır OUAS'ların %100'ü hafife, orta OUAS'ların %77,8'i hafife, %22,2'si normale, hafif OUAS'ların %75'i normale döndü, %25'i hafif OUAS olarak devam etti. Sonuç: Çalışmamız sonucunda AR tanısı alan ve OUAS semptomu bulunan 35 hastanın %42,8'inde OUAS saptandı. 8 haftalık antiinflamatuar tedavi sonucunda AHİ ve OAHİ'de anlamlı düşüş görüldü. Hastaların izleminde TNSS yüksek olan hastalarda hem yaşam kalitesi ve uyku ölçekleri hem de obstrüktif apne sayısı arasındaki ilişki polisomnografi yapılamayan merkezlerde hastaların tedavi kararında yol gösterici olabilir.
Backround and Aim: In patients with Allergic Rhinitis nasal obstruction is accepted as the main reason for sleep disturbances, therefore it is anticipated that treating nasal airway resistance caused by nasal obstruction may improve sleep quality in these patients. In our research we aim to determine the sleep disturbances in patients by using polysomnography, quality of life and sleep quality questionnaires and to observe whether there is improvement in quality of sleep and life after medical treatment. Material and Methods: We restospectively inspected 35 persistent moderate/severe and intermittent moderate/severe Allergic Rhinitis patients' files who applied to Istanbul Medeniyet University Goztepe Prof. Dr. Suleyman Yalçın City Hospital Pediatric Pulmonology Department and gone through polysomnography study because of respiratory disturbances during sleep. Patients' sociodemographic characteristics (height, weight, gender, body mass index), Total Nasal Symptom Scores (TNSS), Sleep Disturbance Scale for Children (SDSC) scores, Pediatric Rhinoconjunctivitis Quality of Life Questionnaire (PRQLQ) scores and polysomnography results are evaluated. Children who are diagnosed with Obstructive Sleep Apnea Syndrome (OSAS) and used medical treatment for 8 weeks were subjected to control PSG study. Patients' PSG parameters, quality of sleep and quality of life scores before and after treatment were compared. Results: 15 of the 35 children included in our study were female and 20 of them were male. In persistent and intermittent moderate/severe Allergic Rhinitis patients, %42.8 (n=15) of them were diagnosed with OSAS. There weren't any significant differences between patients with and without OSAS in terms of age and gender (respectively p=0,693 , p=0250). In OSAS patients, 4 (%26.6) of them were mild, 10 (%66,6) of them were moderate and 1 (%6.6) were severe OSAS. There was significant positive correlation between TNSS and PRQLQ score, sleepiness score and number of sleep apneas. There were significant improvement on quality of life, sleepiness and TNS scores after 8 weeks of medical AR treatment in OSAS patients (p<0.001). When the control polysomnography data of the patients with OSAS were compared after the treatment, a significant decrease was found in the apnea-hypopnea index (AHI) and the obstructive apnea-hypopnea index (OAHI) after the treatment (p<0.001). After the treatment %100 of the severe OSAS patients regressed to mild, %77.8 of the moderate OSAS patients regressed to mild and %22.2 of them recovered completely, %75 of the mild OSAS patients recovered completely and %25 of them carried on as mild OSAS. Conclusion: As a result of our study, OSAS was detected in 42.8% of 35 patients who were diagnosed with AR and had OSAS symptoms. After 8 weeks of anti-inflammatory treatment, a significant decrease was observed in AHI and OAHI. The relationship between both quality of life and sleep scales and the number of obstructive apnea in patients with high TNSS in the follow-up of the patients may guide the treatment decision of the patients in centers where polysomnography cannot be performed.
