Prediyabetli bireylerde egzersizin gdf-15 düzeylerine etkisi

Özet
Amaç: GDF-15 (Growth differentiation factor-15), prediyabette artan inflamatuar bir sitokindir ve anoreksijenik etkileriyle tanınır. Bu çalışma, prediyabetli bireylerde 12 haftalık egzersiz programının GDF-15 üzerine etkilerini değerlendirmeyi amaçlamaktadır. Metod: Bu çok merkezli, çift kör, paralel gruplu, randomize-kontrollü çalışmada, 08.04.2021-07.12.2022 tarihleri arasında, 18-60 yaş arası prediyabet tanısı almış 64 hasta, 1:1 oranında egzersiz (E) ve kontrol (K) gruplarına randomize edilmiştir. Metformin başlaması planlanan 32 hasta da metformin (M) grubuna alınmıştır. Egzersiz grubundaki katılımcılara, 12 hafta boyunca haftanın 3 günü, 60 dakika süresince, maksimum kalp hızlarının %50-70'inde aerobik egzersiz yaptırılmıştır. Çalışmanın başında ve 12. haftanın sonunda egzersiz, kontrol ve metformin gruplarının serum GDF-15 seviyeleri değerlendirilmiştir. Bulgular: Araştırmayı tamamlayan 91 katılımcının (E n:30, K n:30, M n:31) yaş ortalaması 46.13 ± 8.52 olup, %23.1'i erkekti. Başlangıçtaki GDF-15 seviyeleri gruplar arasında benzerdi (E: 668.60±415.11, K: 651.84±352.45, M: 603.64±387.19, p:0.466). On ikinci haftada, egzersiz grubunda kontrol grubuna göre daha düşük, metformin grubunda ise kontrol grubuna göre daha yüksek GDF-15 seviyeleri tespit edildi (E: 383.06±215.63, K: 556.42±285.60, M: 810.80±497.97, p<0.0001). Grup içi karşılaştırmalarda, kontrol grubunda 0. ve 12. haftada anlamlı bir değişiklik olmamışken, egzersiz grubunda GDF-15 seviyeleri başlangıca göre azalmış (p<0.0001) ve metformin grubunda ise artmıştı (p<0.0001). Sonuç: Sonuç olarak, 18-60 yaş arası prediyabetli bireylerde, hem bir metabolik bozukluk biyobelirteci olan hem de iştah üzerinde negatif regülatör etkisi bulunan GDF-15, 12 haftalık aerobik egzersiz uygulanması ile azalmış, metformin verilmesi ile artmıştır. Bu çalışma, GDF-15'in prediyabet yönetimindeki mekanizmaları aydınlatmaktadır. GDF-15'in terapötik potansiyelini belirlemek için daha geniş tabanlı, uzun dönemli çalışmalara ihtiyaç vardır.
Objective: Growth differentiation factor-15 (GDF-15) is an inflammatory cytokine that increases in prediabetes and is known for its anorexigenic effects. This study aims to assess the effects of a 12-week exercise program on GDF-15 in prediabetic individuals. Method: In this multicenter, double-blind, parallel group, randomized-controlled study, between 08.04.2021-07.12.2022, 64 patients aged 18-60 years diagnosed with prediabetes were randomized into exercise (E) and control (C) groups in a 1:1 ratio. Additionally, 32 patients planned to start on metformin were included in the metformin (M) group. Participants in the exercise group were subjected to aerobic exercise at 50-70% of their maximum heart rate, three days a week, for 60 minutes. Serum GDF-15 levels in the exercise, control, and metformin groups were evaluated at the start of the study and at the end of the 12th week. Results: Of the 91 participants who completed the study (E n:30, C n:30, M n:31), the mean age was 46.13 ± 8.52, and 23.1% were male. Initial GDF-15 levels were similar across groups (E: 668.60±415.11, C: 651.84±352.45, M: 603.64±387.19, p=0.466). At week 12, the exercise group had lower GDF-15 levels compared to the control group, while the metformin group had higher levels compared to the control group (E: 383.06±215.63, C: 556.42±285.60, M: 810.80±497.97, p<0.0001). In intra-group comparisons, there was no significant change in the control group between weeks 0 and 12, while GDF-15 levels decreased from baseline in the exercise group (p<0.0001) and increased in the metformin group (p<0.0001). Conclusion: In conclusion, in individuals aged 18-60 with prediabetes, GDF-15, a biomarker of metabolic disorder and a negative regulator of appetite, decreased with 12 weeks of aerobic exercise and increased with the administration of metformin. This study illuminates the mechanisms of GDF-15 in the management of pre-diabetes and highlights the need for broader, longer-term studies to determine the therapeutic potential of GDF-15.
