Topuk kanı alma işlemi sırasında uygulanan farklı yöntemlerin yenidoğanın ağrı düzeyi, ağlama süresi ve işlem süresi üzerine etkisi: Randomize kontrollü çalışma

Bu çalışma bebeklerde topuk kanı alma işlemi sırasında uygulanan vibrasyon ve ShotBlocker yöntemlerinin yenidoğanın ağrı düzeyi, ağlama süresi ve işlem süresi üzerine etkisinin belirlenmesi amacıyla yapıldı. Çalışma, bir eğitim ve araştırma hastanesinin doğum sonu kliniğinde 25 Mayıs 2022-13 Temmuz 2023 tarihleri arasında randomize kontrollü deneysel olarak yapıldı. Araştırmanın örneklemini çalışmanın yapıldığı tarihler arasında klinikte yatan ve araştırma grubu seçim kriterlerine uyan 108 bebek oluşturdu. Örneklem büyüklüğü için yapılan güç analizinde 0.05 anlamlılık seviyesinde, 0.95 güven aralığında ve 0.485 etki büyüklüğü ile araştırmanın gücünün 0.99 olduğu belirlendi. Kontrol grubundaki yenidoğanlara işlemi klinik prosedürlere göre yapıldı. Vibrasyon grubuna topuk delme işleminden önce işlem yapılan ekstremiteye vibratör cihazı yerleştirildi ve 30 sn boyunca vibrasyon uygulandı. ShotBlocker grubuna işlem öncesi topuk delme işlemi yapılacak bölgeye Shotblocker ile cilde baskı uygulandı ve topuk delme işlemi gerçekleştirildi. Veriler Yenidoğan ve Ebeveyn Tanıtıcı Bilgi Formu, İzlem Formu ve Yenidoğan Bebek Ağrı Ölçeği ile toplandı. Veriler, SPSS 25.0 paket programında uygun testler ile analiz edildi. Araştırmanın yapılabilmesi için etik onay, resmi izin ve ailelerden aydınlatılmış onam alındı. Çalışmada işlem sırası, işlemden 2 dk ve 5 dk sonrasında vibrasyon grubundaki yenidoğanların ağrı düzeyinin kontrol ve ShotBlocker grubundan daha düşük olduğu saptandı (p<0.05). Vibrasyon grubundaki yenidoğanların ağlama süresinin kontrol ve ShotBlocker grubundan daha düşük olduğu belirlendi (p<0.05). Grupların topuk kanı alma işlem süresinin benzer olduğu tespit edildi (p>0.05). Yenidoğanlarda oldukça ağrılı bir girişim olan topuk kanı alma işlemi sırasında bebeğin ağrı düzeyini azaltmak amacıyla vibrasyon yönteminin etkili bir yöntem olduğu bulunmuştur. Topuk kanı alma işlemi sırasında kullanılması önerilir.
This study was carried out to determine the effects of vibration and ShotBlocker methods applied during heel blood collection in infants on the level of pain, crying duration and duration of the procedure. The study was conducted as a randomized controlled study in the postpartum clinic of a training and research hospital between May 25, 2022 and July 13, 2023. The sample of the study consisted of 108 infants who were hospitalized in the clinic between the dates of the study and met the research group selection criteria. In the power analysis for the sample size, it was determined that the power of the study was 0.99 with a significance level of 0.05, a confidence interval of 0.95 and an effect size of 0.485. No intervention was performed on the newborns in the control group. Before the heel piercing, a vibrator device was placed on the treated extremity in the vibration group and vibration was applied for 30 seconds. Before the procedure, the ShotBlocker group applied pressure to the skin on the area to be heel piercing, and the heel piercing was performed. Data were collected with the Newborn and Parent Descriptive Information Form, the Follow-up Form, and the NIPS. The data were analyzed with appropriate tests in the SPSS 25.0 package program. Ethical approval, official permission and informed consent from the families were obtained to conduct the study. In the study, it was determined that the pain level of the newborns in the vibration group during the procedure, 2 minutes and 5 minutes after the procedure was lower than the control and ShotBlocker groups (p<0.05). It was determined that the crying duration of the newborns in the vibration group was lower than that of the control and ShotBlocker groups (p<0.05). It was determined that the heel blood collection procedure time of the groups was similar (p>0.05). It has been found that the vibration method is an effective method in order to reduce the baby's pain level during the heel blood collection procedure, which is a very painful procedure in newborns.
