Yoğun bakımda takip edilen COVID-19 pnömonisi hastalarında akciğer ultrason skoru ve mortalite ilişkisi

| dc.description.abstract | Amaç: Akciğer ultrasonografisi (AUS) yatak başı yapılabilen, non-invaziv, kolayca tekrarlanabilir, radyasyon maruziyetinin olmadığı bir teknik olarak, hasta yönetiminde giderek artan şekilde kullanılmaktadır. Bu çalışmanın amacı, toraks üzerinde farklı alanlar değerlendirilerek elde edilen AUS skoru ile yoğun bakımda takip edilen Covid-19 pnömonili hastaların mortalite ilişkisinin araştırılması ve prognostik değerinin gösterilmesidir. Gereç ve Yöntem: Çalışmamızda İstanbul Medeniyet Üniversitesi Göztepe Eğitim Araştırma Hastanesinde yoğun bakım yatış onamları alınan ve 18 yaş üzeri, Covid-19 tanısı nazofarengeal veya bronşial solunum yolu örneklemesinde PCR testi ile doğrulanmış, pnömoni nedeniyle takip edilen hastalardan, yoğun bakım takibi ihtiyacı gelişen 50 hastanın, ünitemize kabul edildikten sonraki 24 saat içinde transtorasik AUS skorları hesaplanmıştır. Hastaların AUS skorları ile demografik verileri, APACHE 2 skorları , ferritin , D-dimer, CRP, WBC , arter kan gazı değerleri, ventilasyon durumları ve 28 günlük mortalite durumları arasındaki ilişki istatistiksel anlamlılık açısından değerlendirilmiştir. Bulgular: Çalışmaya katılan 50 hastada mortalite oranı % 62 (n=31) olarak görülmüştür. Kaybedilen olguların AUS skoru, sağ kalan olgulara göre istatistiksel olarak anlamlı düzeyde yüksek saptanmıştır (p<0,05). Mortaliteye göre AUS skoru için cut off noktası 30 ve üzeri olarak saptanmıştır. AUS skorunun 30 kesme değeri için; duyarlılık %80,65, özgüllük %84,21, pozitif kestirim değeri %89,30 ve negatif kestirim değeri %72,7'dir. Elde edilen ROC eğrisinde altta kalan alan %83,6 standart hatası %6,2 olarak saptanmıştır. Mortalite ile AUS skorunun 30 kesme değeri arasında istatistiksel olarak anlamlı ilişki saptanmıştır ( p<0,01). Kaybedilen olgularda AUS skorunun 30 ve üzerinde görülme riski 22,222 kat fazla bulunmuştur. Çalışmaya katılan olguların AUS skoru ile APACHE-2 skoru, ve SOFA skoru arasında pozitif yönlü ilişki istatistiksel olarak anlamlı bulunmuştur. Çalışmaya katılan olguların AUS skoru ile pH, pCO2, HCO3, laktat, CRP, ferritin, WBC, D-Dimer, albümin, LDH ve prokalsitonin ölçümleri arasında istatistiksel olarak anlamlı ilişki saptanmamıştır (p>0,05). Sonuç: Covid-19 pnömonisi ile yoğun bakımda takip edilen hastaları AUS skoru ile değerlendirdiğimiz bu çalışmada, kaybedilen olguların AUS skoru sağ kalanlara göre istatistiksel olarak anlamlı düzeyde yüksek, APACHE 2, SOFA ve PaO2/FiO2 oranları ile skor arasındaki ilişki de istatistiksel olarak anlamlı bulunmuştur. Bu bulgular ile AUS skorunun yoğun bakımdaki Covid-19 hastalarının prognozunu öngörmede başarılı olduğu, AUS'nun hasta yönetiminde ve akciğer tutulumun derecesinin belirlemesi hakkında klinisyene değerli bilgiler verdiği kanaatine varılmıştır. | |
| dc.description.abstract | Objective: Transthoracic lung ultrasonography is increasingly used in patient management, as a bedside non-invasive, easily reproducible technique without radiation exposure. The aim of this study is to investigate the mortality relationship and to show the prognostic value of patients with covid-19 pneumonia, who are followed up in the intensive care unit with the lung ultrasound score obtained by evaluating different areas on the thorax. Materials and Methods: In our study, after admission to our unit, 50 patients over the age of 18, whose intensive care unit consents were obtained at Istanbul Medeniyet University Göztepe Training and Research Hospital, the diagnosis of Covid-19 was confirmed by PCR test in nasopharyngeal or bronchial respiratory tract sampling, and followed up due to pneumonia, and who developed a need for intensive care follow-up. Transthoracic lung ultrasonography scores were calculated within 24 hours after admisson. The relationship between patients' lung ultrasound (LUS) scores and demographic data, APACHE 2 scores, ferritin, D-dimer, CRP, WBC, arterial blood gas values, ventilation status, and 28-day mortality were evaluated for statistical significance. Results: The mortality rate was 62% (n=31) in 50 patients who participated in the study. The LUS score of the non-survivors was found to be statistically significantly higher than the survivors (p <0.05). The cut-off point for LUS score according to mortality was determined as 30 and above. For the 30 cut-off value of the LUS score; sensitivity 80.65%, specificity 84.21%, positive predictive value 89.30% and negative predictive value 72.7%. The area under the ROC curve obtained was 83.6% with a standard error of 6.2%. A statistically significant correlation was found between mortality and the 30 cut-off value of the LUS score (p<0.01). The risk of having an LUS score of 30 and above was found to be 22,222 times higher in lost cases. A positive correlation between LUS score, APACHE-2 score, and SOFA score of the subjects participating in the study was statistically significant. No statistically significant correlation was found between the LUS score of the subjects participating in the study and the measurements of pH, pCO2, HCO3, lactate, CRP, ferritin, WBC, D-Dimer, albumin, LDH and procalcitonin (p>0.05). Conclusion: In this study, in which we evaluated the lung ultrasound score of patients who were followed up in the intensive care unit with Covid-19 pneumonia, the lung ultrasound score of non-survivors was statistically significantly higher than the survivors, and the relationship between the APACHE 2, SOFA and PaO2/FiO2 ratios and the score was also found to be statistically significant. With these findings, we concluded that by evaluating the lung ultrasound score, meaningful data can be obtained regarding the prognosis of intensive care patients followed up with covid-19 pneumonia and can be used in patient management. | |
