Yenidoğanlarda sarılık en sık tedavi gerektiren hastaneye başvuru nedenlerinden biridir. Klinik uygulamada yenidoğan sarılığının tedavisinde kullanılan yöntem fototerapidir. Çalışmada yenidoğan bebeklerde indirekt hiperbilirubinemi tedavisinde uygulanan devamlı fototerapi modeli ile aralıklı fototerapi modeli etkinlik ve yan etki insidansı açısından karşılaştırıldı. Tek merkezli, prospektif ve randomize bir çalışma olarak tasarlandı. Temmuz 2024- Haziran 2025 tarihleri arasında yenidoğan yoğun bakım ünitemizde yatarak fototerapi tedavisi uygulanan ?34 gestasyon haftasında doğan yenidoğan bebekler çalışmaya dahil edildi. Çalışmada randomizasyon için bilgisayar tabanlı rastgele sayılar tablosu kullanıldı. Fototerapi uygulama şekline göre devamlı ve aralıklı fototerapi olarak iki modele ayrıldı. Devamlı fototerapide 6 saat kesintisiz bir kür ve aralıklı fototerapide 30 dakika açık, 90 dakika kapalı üç döngü ile 6 saatlik bir kür uygulandı. Fototerapi tedavisi başlanğıcında ve kür sonunda total serum bilirubin, hematokrit değerleri ölçüldü. Her iki modelde de bir kür fototerapi uygulandıktan 8-12 saat sonra rebound bilirubin bakıldı. Uygulanan tedavi modelleri fototerapi süresi, total serum bilirubin azalma hızı, rebound görülen hasta oranı, rebound artış miktarı, tedavi sonu kür sayısı, hastanede yatış süresi gibi fototerapi etkinliğini gösteren parametreler açısından karşılaştırıldı. Çalışma süresince 156 yenidoğan sarılık nedeniyle ünitemize yatırılarak fototerapi tedavisi aldı. 76 yenidoğan dahil etme kriterlerine uymadığı için çalışma dışı bırakıldı. Çalışmaya dâhil edilen toplam 80 yenidoğan randomizasyonla eşit olarak devamlı fototerapi veya aralıklı fototerapi gruplarına atandı. Çalışmada cinsiyet, doğum şekli, doğum ağırlığı, yatış ağırlığı, postnatal başvuru yaşı, taburcu ağırlığı ve kardeşte fototerapi öyküsü gibi demografik özellikleri iki grup arasında benzer bulundu (p>0,05). Hastaların %53,75'i erkek olup, %46,25'i kızdı. Bu kohortda yenidoğan sarılığının en sık nedeni güncel laboratuvar testleri kullanılarak patolojik bır neden bulunamayan idiyopatik grupdu. Fototerapi öncesinde bakılan hematokrit ve başvuru bilirubin değerleri benzer özellikteydi (sırasıyla p=0.899 ve 0.0832). Yatış süresinin ortanca değeri devamlı fototerapi uygulananlarda 6 (3–8)gün, aralıklı fototerapi uygulananlarda 4 (3–5) gün olarak istatistiksel açıdan anlamlı bulunmadı (p=0,088). Fototerapi süresinin ortanca değeri devamlı fototerapi grubunda 6 (6-12) saat, aralıklı fototerapi grubunda 1,5 (1,5-3) saatti (p<0,001). Bilirubin azalma hızı aralıklı fototerapi grubunda 2,23?± 1,58 mg/dL/sa iken, devamlı fototerapi grubunda bu değer 0,62??± 0,52 mg/dL/saat olarak saptandı (p<0,001). Çalışma grupları arasında rebound bilirubin artış miktarının ortanca değeri devamlı fototerapi grubunda -0.45 mg/dL/sa ve aralıklı fototerapi grubunda ise -1.20 mg/dL/saatti (p=0.283). Devamlı fototerapi ve aralıklı fototerapi grupları yan etkiler açısından karşılaştırıldığında: vücut sıcaklık değişimi, vücut tartı yüzdesi, dışkılama sayısı ve cilt döküntüsü görülme sıklığı açısından benzerdi (p>0,005). Yenidoğan sarılığının tedavisinde etkinlik bakımından aralıklı ve devamlı fototerapinin bir birine üstünlüğü olmadığı gösterilmiştir. Fototerapi ışık maruziyet süresini azaltan aralıklı model güncel fototerapi uygulamalarında akılcı bir tedavi olarak düşünülmelidir.
Neonatal jaundice is one of the most common causes of hospital admissions requiring treatment in newborns. In clinical practice, phototherapy is the standard method used for the management of neonatal jaundice. This study compared the efficacy and incidence of side effects between continuous and intermittent phototherapy models in the treatment of indirect hyperbilirubinemia in neonates. This was a single-center, prospective, randomized study. Between July 2024 and June 2025, neonates born at a gestational age of ?34 weeks and hospitalized in our neonatal intensive care unit for phototherapy were included. Randomization was performed using a computer-generated random number table. According to the mode of administration, patients were allocated to either continuous or intermittent phototherapy groups. In the continuous phototherapy model, a 6-hour uninterrupted session was applied, whereas in the intermittent model, three cycles of 30 minutes on and 90 minutes off were administered over a 6-hour session. Total serum bilirubin and hematocrit levels were measured at baseline and at the end of each session. Rebound bilirubin levels were checked 8–12 hours after the completion of one phototherapy session in both groups. The treatment models were compared in terms of phototherapy duration, bilirubin reduction rate, incidence and magnitude of rebound bilirubin, number of phototherapy sessions, and length of hospital stay. During the study period, 156 neonates were admitted to our unit with jaundice and received phototherapy. Seventy-six did not meet the inclusion criteria and were excluded. A total of 80 neonates were included and randomized equally into continuous and intermittent phototherapy groups. Baseline demographic characteristics, including sex, mode of delivery, birth weight, admission weight, postnatal age at admission, discharge weight, and history of sibling phototherapy, were similar between the groups (p>0.05). Of the participants, 53.75% were male and 46.25% were female. The most common etiology of neonatal jaundice in this cohort was idiopathic, with no pathological cause identified through current laboratory testing. Baseline hematocrit and bilirubin levels were comparable between groups (p=0.899 and p=0.0832, respectively). The median length of hospital stay was 6 (3–8) days in the continuous group and 4 (3–5) days in the intermittent group, with no statistically significant difference (p=0.088). Median phototherapy duration was significantly longer in the continuous group [6 (6–12) hours] compared to the intermittent group [1.5 (1.5–3) hours] (p<0.001). The bilirubin reduction rate was higher in the intermittent group (2.23 ± 1.58 mg/dL/h) compared to the continuous group (0.62 ± 0.52 mg/dL/h) (p<0.001). The median rebound bilirubin increase was -0.45 mg/dL/h in the continuous group and -1.20 mg/dL/h in the intermittent group, with no significant difference (p=0.283). The incidence of side effects—including changes in body temperature, percentage weight loss, frequency of defecation, and skin rash—was similar between the two groups (p>0.005). No superiority was demonstrated between continuous and intermittent phototherapy in terms of overall efficacy in the treatment of neonatal jaundice. However, the intermittent model, which reduces total light exposure, may be considered a rational approach in contemporary phototherapy practice.
