Özet

Diffuz Büyük B hücreli lenfoma (DBBHL), erişkin non-Hodgkin lenfomaları( NHL) içinde en yaygın alt tip olup, vakaların yaklaşık %40'ını oluşturur. R-CHOP immünoterapisiyle tedavi edilen hastaların progresyonsuz sağkalımı % 60-70 civarındadır. Buna karşın hastaların %30–40 kadarı ilk tedaviye yanıt vermemekte veya relaps gelişmektedir. Mevcut prognostik skorlar (IPI, R-IPI, NCCN-IPI) tedaviye yanıtı iyi öngörse de yüksek risk grubunu yeterince ayırt edemeyebilmektedir. Bu bağlamda, tümör biyolojisini, inflamasyonu ve beslenme durumunu yansıtan ek biyobelirteçlerin tanımlanması önem kazanmıştır. CONUT skoru (Controlling Nutritional Status), serum albümin, total lenfosit sayısı ve total kolesterol düzeylerine dayanan, hastanın malnutrisyon ve immün durumunu yansıtan bir beslenme skorlama sistemidir. Yapılan çalışmalar, yüksek CONUT skorunun çeşitli solid tümörlerde kötü prognozla ilişkili olduğunu ve DBBHL'de de benzer bir etki gösterebileceğini öne sürmüştür. Bu çalışmada merkezimize son 10 yılda başvuran 99 DBBHL hastasının tanı öncesi CONUT skorları hesaplanarak genel sağkalım (OS) ve progresyonsuz sağkalım (PFS) ile ilişkisi değerlendirildi. Veriler yaş, evre, B semptomları, ECOG performans durumu, sitogenetik bulgular, tedavi protokolü gibi klinik özelliklerle birlikte incelendi. Tek değişkenli ve çok değişkenli Cox regresyon analizlerinde yüksek CONUT skorunun OS ve PFS üzerinde olumsuz etkisi araştırıldı. Analiz sonuçlarına göre, yüksek CONUT skoruna sahip hastaların genel sağkalım ve progresyonsuz sağkalım süreleri anlamlı olarak daha kısaydı. Çalışmamızda çok değişkenli analizde yüksek CONUT skoru bağımsız kötü prognoz belirteci olarak öne çıktı. Genel sağkalım açısından yapılan tek değişkenli analizlerde CONUT skoru (HR: 1,414; %95 GA:? 1,244–1,608; p<0,001) artışı ölüm riski ile anlamlı şekilde ilişkili bulundu. 5 yıllık PFS CONUT skoru ile değerlendirildiğinde, normal beslenme grubunda %69,0 iken, ağır malnütrisyon grubunda hiçbir hastada 1 yılın ötesinde hastalıksız sağkalım gözlenmedi. Progresyonsuz sağkalım açısından değerlendirildiğinde CONUT skoru en yüksek CPE değeriyle (0,661; %95 GA: 0,525–0,798) en iyi tahmin gücünü sergiledi ve bu da olay tahmin performansı açısından bu değişkenin daha etkili olduğunu düşündürdü. Bulgularımız literatürle uyumlu olarak, CONUT skorunun DLBHL'de sağkalımı öngörmede anlamlı katkı verdiğini ortaya koydu. CONUT skorunun klinik kullanımı pratik ve düşük maliyetlidir. Skorun yüksek olması hastanın beslenme durumunun kötü olduğunu, immün yanıt kapasitesininin düşük olduğunu ve de agresif tümör biyolojisini gösterir. Çalışmamız ve diğer veriler, CONUT'un geleneksel skorlara ek olarak yüksek riskli hastaları belirlemede yardımcı olabileceğini göstermiştir. Bu bağlamda, DBBHL tanısında CONUT skorunun tedavi stratejilerinin planlanmasında bir tamamlayıcı araç olarak kullanılması önerilebilir. Özetle, CONUT skorunun DBBHL hastalarında hem tümörün klinik yaygınlığı hem de beslenme-inflamasyon durumu hakkında bütüncül bilgi veren güçlü bir prognostik belirteç olarak kullanılması çalışılmaktadır.

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of adult non-Hodgkin lymphomas (NHL), accounting for approximately 40% of all cases. The progression-free survival (PFS) rate in patients treated with R-CHOP chemoimmunotherapy is approximately 60–70%. However, around 30–40% of patients either do not respond to first-line treatment or experience disease relapse. Although current prognostic scoring systems such as the International Prognostic Index (IPI), Revised IPI (R-IPI), and the National Comprehensive Cancer Network IPI (NCCN-IPI) are effective in predicting treatment response, they may not sufficiently distinguish high-risk patient groups. In this context, the identification of additional biomarkers reflecting tumor biology, systemic inflammation, and nutritional status has gained importance. The CONUT (Controlling Nutritional Status) score is a nutritional assessment tool based on serum albumin level, total lymphocyte count, and total cholesterol concentration, reflecting the patient's nutritional and immune status. Several studies have shown that a high CONUT score is associated with poor prognosis in various solid tumors, and it has been suggested that it may have a similar prognostic impact in DLBCL. In this study, we retrospectively evaluated the pre-treatment CONUT scores of 99 patients diagnosed with DLBCL at our center over the past 10 years and investigated their association with overall survival (OS) and progression-free survival (PFS). Clinical variables such as age, disease stage, presence of B symptoms, ECOG performance status, cytogenetic findings, and treatment protocols were also analyzed. The impact of a high CONUT score on OS and PFS was assessed using univariate and multivariate Cox regression analyses. According to the results, patients with higher CONUT scores had significantly shorter OS and PFS durations. In our multivariate analysis, a high CONUT score emerged as an independent predictor of poor prognosis. In univariate analysis for overall survival, an increase in CONUT score was significantly associated with a higher risk of mortality (HR: 1.414; 95% CI: 1.244–1.608; p < 0.001). When PFS was evaluated by CONUT score, 5-year PFS was 69.0% in the normal nutrition group, whereas no patients in the severe malnutrition group survived disease-free beyond one year. Regarding PFS prediction, the CONUT score demonstrated the highest concordance probability estimate (CPE = 0.661; 95% CI: 0.525–0.798), indicating that it had the strongest predictive power among the parameters evaluated. Our findings, consistent with the literature, demonstrate that the CONUT score provides significant additional value in predicting survival in DLBCL. The clinical use of the CONUT score is practical and low-cost. A high score reflects poor nutritional status, diminished immune capacity, and possibly more aggressive tumor biology. Our study and supporting data suggest that the CONUT score can aid in identifying high-risk patients when used alongside conventional prognostic systems. In this context, the CONUT score may be proposed as a complementary tool in planning treatment strategies for patients diagnosed with DLBCL. In summary, the CONUT score is being evaluated as a strong prognostic marker that provides comprehensive information on both the clinical extent of disease and the patient's nutritional-inflammatory status in DLBCL.
