Tekrarlayan alt solunum yolu enfeksiyonu ve kronik akciğer hastalığı olan çocuklarda yeni nesil dizilemenin uygulanması

| dc.description.abstract | Çocukluk çağında tekrarlayan alt solunum yolu enfeksiyonları ve kronik akciğer hastalığı hastane başvurularının önemli bir nedenini oluşturmaktadır. Fiziksel muayene, biyokimyasal / mikrobiyolojik tahliller ve görüntüleme çalışmalarına rağmen, bu klinik durumların teşhisi hala zordur. Bu çalışmanın amacı, şüpheli genetik akciğer hastalıkları olan pediyatrik grupta yeni nesil dizilemenin (YND) tanıya katkısını değerlendirmek ve klinik özellikler ile genetik bulgular arasındaki dağılımı ve ilişkileri analiz etmektir. Çalışmamıza 01.09.2020-01.06.2023 tarihleri arasında hastanemizde değerlendirilen şüpheli genetik hastalığı olan toplam 198 çocuk dahil edildi. Bunlar arasında 137 hastaya klinik ekzom dizileme (CES) uygulanırken, 63 hastaya tüm ekzom dizileme (WES) uygulandı (WES, CES ile değişim tespit edilmeyen iki hastaya da uygulandı). Hedef bölgeler CES için Clinical Exome Solution by Sophia Genetics kiti, WES için Twist_Hcexome_V2 kiti kullanılarak çoğaltıldı ve dizileme Illumina NextSeq® platformunda gerçekleştirildi. Hastaların klinik bulguları ile ilişkili olduğu düşünülen patojenik (P), muhtemel patojenik (MP) ve klinik önemi bilinmeyen değişimler (VUS) ACMG (Amerikan Tıbbi Genetik ve Genomik Koleji) sınıflamasına göre değerlendirildi. Bu çalışmada 80 (%40,41) kız ve 118 (%59,59) erkek olmak üzere toplam 198 hasta değerlendirildi. Ortalama başvuru yaşı 6,24±4,78 yıldı. 61 hastada (%30,8) hastalıkla ilişkili değişimler saptanırken, 137 hastada (%69,2) değişim saptanmadı. YND ile saptanan tanılar arasında 21 hastada (%10,6) primer siliyer diskinezi, 13 hastada (%6,56) kistik fibrozis, 10 hastada (%5,05) primer immün yetmezlik, dokuz hastada (%4,54) astım/atopi yatkınlığı, dört hastada (%2,02) interstisyel akciğer hastalığı, üç hastada (%1,51) heterotaksi, bir hastada (%0,5) metabolik hastalık, bir hastada (%0,5) bronşektazi vardı. Ek olarak, YND' de değişim saptanan grupta; bronşektazi, nazal polip ve akraba evliliği öyküsü değişim saptanmayan gruba göre anlamlı derecede daha yüksekti (p<0,05). Sonuç olarak, bu retrospektif tek merkezli çalışma YND teknolojilerinin tekrarlayan alt solunum yolu enfeksiyonları ve kronik akciğer hastalığı olan pediyatrik hastaların tanısına olan katkısını vurgulamaktadır. Genetik tanı koymak; iyileştirilmiş tedavi stratejileri, genetik danışmanlık ve implantasyon öncesi genetik tanı dahil olmak üzere önemli klinik faydalar sağlar. Ayrıca genotip-fenotip dağılımlarını ve bunların potansiyel ilişkilerini daha iyi anlamak için kapsamlı, çok merkezli popülasyon çalışmaları gereklidir. Gelecekteki tüm genom dizileme (WGS) çalışmaları, yeni/aday genlerin keşfini kolaylaştıracak ve YND' nin değişim tespit oranını artıracaktır. | |
| dc.description.abstract | Recurrent lower respiratory tract infections and chronic lung disease in childhood constitute a significant cause of hospital admissions. Despite physical examinations, biochemical/ microbiological analyses, and imaging studies, diagnosing these clinic conditions remains challenging. This study aims to evaluate the contribution of next-generation sequencing (NGS) in diagnosing pediatric grup with suspected genetic lung diseases and to analyze the distribution and relationships between clinical features and genetic findings. Between 01.09.2020-01.06.2023, a total of 198 children with suspected genetic disease, who were evaluated in our hospital, were included in our study. Among them, 137 patients underwent clinical exome sequencing (CES), while 63 patients underwent whole-exome sequencing (WES) (WES also was performed in two patients after no variants were detected through CES). Target regions were amplified using the Clinical Exome Solution by Sophia Genetics kit for CES, the Twist_Hcexome_V2 kit for WES and sequencing was performed on the Illumina NextSeq® platform. Variants classified as pathogenic (P), likely pathogenic (LP), or variant of uncertain significance (VUS), which were considered relevant to the patients' clinical findings, were evaluated according to the ACMG (American College of Medical Genetics and Genomics) classification. In this study, a total of 198 patients, 80 (40.41%) female and 118 (59.59%) male, were evaluated. The mean age at admission was 6,24±4,78 years. Disease-associated variants were identified in 61 patients (30.8%), while no variants were detected in 137 patients (69.2%). The diagnoses identified through NGS included primary ciliary dyskinesia in 21 patients (10.6%), cystic fibrosis in 13 patients (6.56%), primary immunodeficiency in 10 patients (5.05%), asthma/atopy tendency in nine patients (4.54%), interstitial lung disease in four patients (2.02%), heterotaxy in three patients (1.51%), metabolic disease in one patient (0.5%), and bronchiectasis in one patient (0.5%). Additionally, a history of bronchiectasis, nasal polyps and consanguineous marriages was significantly higher in the group with variants detected in NGS compared to the group without variants detected (p<0.05). In conclusion, this retrospective single-center study highlights the contribution of NGS technologies to the diagnosis of pediatric patients with recurrent lower respiratory tract infections and chronic lung disease. Establishing a genetic diagnosis provides significant clinical benefits, including improved treatment strategies, genetic counseling and pre-implantation genetic diagnosis. Furthermore, comprehensive, multicenter population studies are required to better understand the genotype-phenotype distributions and their potential relationships. Future whole-genome sequencing (WGS) studies will facilitate the discovery of novel/candidate genes and enhance the variant detection rate of NGS. | |
