Preterm bebeklerde anne sütü ile beslenme ve major morbiditelerle ilişkisi

| dc.description.abstract | Son yıllardaki gelişmelerle birlikte yaşam şansları artan küçük prematüre bebekler prematüriteye özgü major morbiditeler açısından en riskli grubu oluşturmaktadır. Sayısız biyoaktif komponenti barındıran anne sütü (AS) ise bu bebeklerin sağlığını olumlu yönde değiştirmekte, özellikle de nekrotizan enterokolit (NEK)'e karşı güçlü bir koruma sağlamaktadır. Çalışmamız 34. gestasyon haftasından (GH) önce doğan prematüre bebeklerde, Yenidoğan Yoğun Bakım Ünitesi'nde yatış sırasında ve taburculukta AS ile beslenme durumunu belirlemek; AS alımının NEK, BPD, ROP ve geç başlangıçlı sepsis insidansı üzerine etkisini ve bu potansiyel etkinin doza bağımlı olup olmadığını saptamaktır. Retrospektif kesitsel olarak planlanan bu çalışmaya; 01.01.2018 ile 31.12.2022 tarihleri arasındaki 5 yıllık sürede en az 28 gün yatarak bakım ve tedavi görmüş, gestasyon yaşı <34 hafta olan prematüre bebekler alınmıştır. Veriler, elektronik kayıt sistemi (Nucleus) yolu ile, gerektiğinde yazılı dosyalara başvurarak toplanmıştır: Bebeklerin doğumdaki demografik özellikleri; anneye ve hastalıklarına ait bilgiler, gestasyonel patolojiler; major komorbiditelere ve beslenmeye ait parametreler; ilk 28 günde günlük, daha sonra haftada bir gün olmak üzere AS alım miktarları; yatış süresi; taburculukta AS alım yüzdesi ve ekstrauterin büyüme kısıtlılığı (EUBK) varlığı. AS'nün morbiditeler üzerine etkisini analiz etmek için AS /enteral alım oranına göre 3 grup tanımlanmıştır. AS grubu: >%75 AS, karışık grup: %25-74 AS, prematüre mama (PM) grubu: %0-25 AS. Bağımlı değişkenlerin karşılaştırılması AS, PM grupları arasında yapılmıştır. AS'nün morbiditeler üzerinde doza bağımlı koruyucu etkisini araştırmak için 2 farklı yöntem kullanılmıştır: 1. AS alım miktarlarına göre gruplayarak, 2. Farklı zaman dilimlerinde AS alımlarında her 10 ml/kg/gün artışın etkisini analiz ederek. Çalışma grubumuzu gestasyon yaşı <34 hafta olan 180 prematüre bebek oluşturmuştur. AS ile beslenme durumu değerlendirildiğinde; YYBÜ'de yatışları sırasında çalışma grubundaki bebeklerin ilk 14 günde %73,3'ünün yalnızca/ağırlıklı olarak AS ile, %15'inin karışık, %11,7'sinin ise PM ile beslendiği saptanmıştır. Taburculukta yalnızca/ağırlıklı AS ile beslenme oranı %58,3'tür. İlk 14 günde; AS grubunda NEK, Evre 2 NEK, cerrahi NEK, BPD, ağır BPD, anti-VGEF gerektiren ROP sıklığı PM grubundan anlamlı olarak daha düşüktür (sırasıyla p<0,001,p=0,034, p<0,001, p<0,040, p=0,026, p=0,005). İlk 28 gün için bu değerlendirme yapıldığında; AS grubunda geç başlangıçlı sepsis (% 37,2 vs %59,4, p=0,041) ve kültür kanıtlı sepsis (%9,7 vs %37,5, p<0,001) sıklığının da azalmış olduğu görülmüştür. AS grubunda yatış süresi, PM grubuna göre daha kısa (p=0,025); EUBK sıklığı ise çok daha düşüktür (p=0,027). AS enteral alımın en az %25'ine ulaştığında NEK, Evre 2 ve cerrahi NEK, %50'sine ulaştığında ise bu koruyucu etkilere ek olarak, BPD ve ağır BPD, anti-VGEF tedavisi gerektiren ROP sıklığının da azaldığı saptanmıştır (sırasıyla p<0,001, p=0,020, p<0,001, p=0,012, p<0,01, p=0,005). AS enteral alımın %75'ine ulaştığında geç başlangıçlı sepsis sıklığı da azalmıştır (p=0,024). İlk 14 gün ve hatta ilk 28 gün boyunca AS alımında her 10 ml/kg/gün artışa karşılık NEK insidansı sırasıyla %1,21 ve %1,15 azalmaktadır (sırasıyla ß=-1,21 ve -1,15, p=0,001 ve p<0,001). En güçlü etki etki postnatal, 1. Haftadadır, her 10 ml/kg/gün artış NEK insidansını %1,36 azaltmaktadır (ß=- 1,36, p=0,004). NEK insidansını azaltıcı etki 8 hafta boyunca devam etmektedir. AS mikarındaki her 10 ml/kg/gün artış postnatal ilk 6 hafta boyunca BPD ve geç başlangıçlı sepsis insidansını da anlamlı şekilde düşürmektedir. En güçlü etki yine 1. haftadadır (ß=- =,54 ve -0,44, p<0,01). Sonuç olarak; çalışmamızda AS'nün <34 gestasyon haftasında doğan prematüre bebekleri YYBÜ'nün en önemli morbiditeleri olan NEK, BPD, ağır BPD, anti-VGEF gerektiren ROP, geç başlangıçlı sepsis ve kanıtlanmış sepsisten doza bağımlı olarak koruyabildiği saptanmıştır. Çalışmamızın farklılığı; AS'nün doza bağlı etkilerinin kapsamlı bir şekilde birden fazla yöntemle ve birden çok morbidite için analiz edilmiş olmasıdır. Her prematüre bebek annesinin sütüne kavuşturulmalı ve devamlılığı sağlanmalıdır. | |
| dc.description.abstract | Small premature babies, whose survival chances have increased with the developments in recent years, constitute the most risky group in terms of majör morbidities spesific to prematurity. Breast milk, which contains numerous bioactive components, positively changes the health of these babies and provides strong protection, especially against necrotizing enterocolitis (NEC). Our study was determine the breastfeeding status of premature babies born before the 34th week of gestation, during hospitalization in the Neonatal Intensive Care Unit and at discharge; to determine the effect of breastfeeding on the incidence of necrotizing enterocolitis, bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP) and late-onset Sepsis and whether this potential effect is dose dependent. This study was planned as a retrospective cross-sectional study; premature babies with a gestational age <34 weeks who received inpatient care and treatment for at least 28 days in the 5-year period between 01.01.2018 and 31.12.2022 were included. Data were collected through the electronic recording system (Nucleus) and by referring to written files when necessary: Demographic characteristics of babies at birth; information about the mother and her diseases, gestational pathologies; parameters of major comorbidities and nutrition; Breast milk intake daily in the first 28 days and once a week thereafter; length of stay; Percentage of breast milk intake at discharge and presence of extrauterine growth restriction. To analyze the effect of breast milk on morbidities, three groups were defined according to the breast milk / enteral intake ratio. breast milk group: >75% breast milk, mixed group: 25-74% AS, premature formula group: <0-25% breast milk. Comparison of dependent variables was made between breast milk and premature formula groups. Two different methods were used to analyze the dose-dependent protective effect of breast milk on morbidities: 1. by grouping according to breast milk intake amounts, 2. By analyzing the impact of each 10 ml/kg/day increase in breast milk intake over different time periods. Our study group consisted of 180 premature babies with a gestational age of <34 weeks. When the nutritional status with breast milk is evaluated; It was determined that during the first 14 days of hospitalization in the NICU, 73.3% of the babies in the study group were fed exclusively/mainly with breast milk, 15% were fed with mixed milk, and 11.7% were fed with premature formula. The rate of exclusive/predominant breastfeeding at discharge is 58.3%. In the first 14 days; the frequency of NEC, Stage 2 NEC, surgical NEC, BPD, severe BPD, and ROP requiring anti-VGEF is significantly lower in the breast milk group than in the premature formula group (p<0.001, p=0.034, p<0.001, p=0.040, p=0.026, p=0.005, respectively). When this evaluation is made for the first 28 days; the frequency of late-onset sepsis (37.2% vs 59.4%, p=0.041) and culture-proven sepsis (9.7% vs 37.5%, p<0.001) was also observed to decrease in the breast milk group. The length of stay in the breast milk group was shorter than in the premature formula group (p=0.025); the frequency of extrauterine growth restriction (EUGR) is much lower (p=0.027). It was determined that when breast milk reached at least 25% of enteral intake, the frequency of NEC, Stage 2 and surgical NEC, and when it reached 50%, in addition to these protective effects, the frequency of BPD and severe BPD and ROP requiring anti-VGEF treatment also decreased (p<0.001, p=0.020, p<0.001, p=0.012, p<0.01, p=0.005). When breast milk reached 75% of enteral intake, the frequency of late-onset sepsis also decreased (p=0.024). For every 10 ml/kg/day increase in breast milk intake during the first 14 days and even the first 28 days, the incidence of NEC decreases by 1.21% and 1.15%, (respectively ß=-1.21 and -1.15, p= 0.001 and p<0.001). The strongest effect is in the postnatal 1st week, each 10 ml/kg/day increase reduces the incidence of NEC by 1.36% (ß=-1.36, p=0.004). The effect of reducing the incidence of NEC continues for 8 weeks. Every 10 ml/kg/day increase in breast milk amount significantly reduces the incidence of BPD and late-onset sepsis during the first 6 postnatal weeks. The strongest effect is again in the 1st week (ß=-=.54 and -0.44, p<0.01). In conclusion; in our study, it was determined that breast milk could protect premature babies born at <34 weeks of gestation from NEC, BPD, severe BPD, ROP requiring anti-VGEF, late-onset sepsis and proven sepsis, which are the most important morbidities of NICU, in a dose-dependent manner. The difference of our study; the dose- related effects of breast milk have been extensively analyzed by multiple methods and for multiple morbidities. Every premature baby should be introduced to its mother's milk and its continuity should be ensured. | |
