Özet

LAPAROSKOPİK KOLESİSTEKTOMİ AMELİYATLARINDA İKİ FARKLI PEEP DEĞERİNİN PULMONER FONKSİYONLARA ETKİLERİNİN ULTRASONOGRAFİ İLE DEĞERLENDİRİLMESİ Atelektazi genel anestezi sırasında, özellikle akciğer problemi olan hastalarda ve pnömoperitoneum uygulanan hastalarda, görülebilecek önemli bir komplikasyondur. Anestezi sırasında PEEP uygulanması atelektazi gelişimini azaltabilir. Akciğer ultrasonografisi kolay erişilebilen, yatak başı uygulanabilen, non-invaziv , radyasyon maruziyeti yaratmayan ve ameliyathanelerde günlük pratikte kullanımı gün geçtikçe yaygınlaşan bir araçtır. Bu çalışmada, genel anestezi alan iki farklı değerde PEEP uygulanan hastalarda, atelektazi gelişiminin akciğer ultrasonu kullanılarak değerlendirilmesi ve ultrasonun atelektazi gelişme riskinin belirlenmesindeki etkinliğinin araştırılması amaçlanmıştır. Çalışmamız İstanbul Medeniyet Üniversitesi Göztepe Eğitim ve Araştırma Hastanesi Genel Cerrahi Kliniğinde planlı laparoskopik kolesistektomi cerrahisi endikasyonu konulan, yazılı ve sözlü bilgilendirilmiş onamları alınmış, yaşları 18 ve daha büyük olan, ASA I-III gruplarından 60 hastada yapılmıştır. Çalışmaya alınan hastalar 2 gruba ayrıldı. Opere olacak hastalardan anestezi cihazında iken Grup I (n=30) olgulara 5 PEEP ve Grup II (n=30) olgulara 10 PEEP uygulandı. Çalışmaya dahil edilen hastaların demografik verileri, ASA skorları , vücut kitle indeksleri, ağırlıkları, anestezi süresi, cerrahi süresi ve pnömoperitoneum süreleri her iki grupta benzerdi.

EVALUATION OF THE EFFECTS OF TWO DIFFERENT PEEP LEVELS ON PULMONARY FUNCTIONS IN LAPAROSCOPIC CHOLECYSTECTOMY SURGERIES BY ULTRASONOGRAPHY Atelectasis is an important complication that can be seen during general anesthesia, especially in patients with lung problems and in patients having pneumoperitoneum. Application of PEEP during anesthesia may reduce the incidence of atelectasis. Lung ultrasonography is a tool that is easily accessible, can be applied at the bedside, is non-invasive, does not cause radiation exposure, and is increasingly used in daily practice in operating rooms. In this study, it was aimed to evaluate the occurrence of atelectasis by using lung ultrasound, and to investigate the effectiveness of ultrasound in determining the risk of developing atelectasis in patients ventilated with different PEEP values during general anesthesia. Our study was conducted in 60 patients aged 18 and over, from ASA I-III groups, who were planned to undergo laparoscopic cholecystectomy surgery at the Istanbul Medeniyet University Göztepe Training and Research Hospital General Surgery Clinic, and their written and verbal informed consents were obtained. The patients included in the study were divided into 2 groups. Among the patients who were going to be operated, 5 PEEP were applied to Group I (n=30) cases, and 10 PEEP to Group II (n=30) cases during ventilation with anesthesia device. All patients were evaluated by transthoracic lung ultrasonography before anesthetic induction, after tracheal intubation, after pneumoperitoneum, and 15 minutes after recovery from anesthesia. According to the lung USG protocol, a total of 6 regions, including 2 quadrants of each hemi-thorax anterior wall, 2 right and left lateral walls, and 2 quadrants of the posterior wall, a total of 12 regions in each patient were scanned, and the images were evaluated. According to the modified lung USG scoring system, each region was scored between 0-3 consistent with the degree of atelectasis. The total score was calculated for each patient. Hemodynamic follow-up of the patients was performed. Anesthesia times, surgery times, and pneumoperitoneum times were recorded and compared. Demographic data, ASA scores, body mass indexes, weights, duration of anesthesia, duration of surgery and pneumoperitoneum duration of the patients included in the study were similar in both groups. While there was no statistically significant difference between the Lung USG scores (LUS) of the groups before general anesthesia (p>0.05); post-intubation, post-pneumoperitoneum and recovery room LUS values of the Group I cases were found to be significantly higher than those obtained in Group II patients (p<0.05). It has been shown with the lung ultrasonography that the patients having pneumoperitoneum and ventilated with higher PEEP during general anesthesia have less aeration loss. We conclude that the application of high PEEP in the perioperative period may reduce the formation of atelectasis, and lung ultrasonography may be useful in the early detection of atelectasis in these patients.
