dc.description.abstractMultipl skleroz (MS), inflamasyon ve nörodejeneratif süreçlerle karakterize, merkezi sinir sisteminin kronik inflamatuvar bir hastalığıdır. MS'de inflamasyon, ısı şoku proteini 70 (HSP70) ve ısı şok poretini 90'nın (HSP90) hücre içerisinde sentezini ve hücre dışı alana salınımını indükler bu da bağışıklık yanıtlarını ortaya çıkmasına katkıda bulunabilir. Bu çalışmanın ana amacı, Relapsing Remitting Multipl Skleroz (RRMS)'de hastalığın inflamasyonun en yoğun olduğu atak döneminde ve hastalığın akut inflamatuvar aktivitesinin gerilediği remisyon dönemindeki serum HSP70, HSP90 seviyelerini saptamak ve bu proteinlerin gen ekspresyon düzeylerini belirleyip atak ve remisyon döneminde hastalığın patogenezindeki rolünü anlayabilmektir. Bu prospektif çalışmaya İstanbul Medeniyet Üniversitesi S.B. Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Nöroloji Multipl Skleroz polikliniğinden takipli olan RRMS tanılı 18-50 yaş aralığında 40 hasta (ortalama yaş; 33,55±10,04, 26 Kadın), yaş ve cinsiyet açıcından eşleştirilmiş 40 kişilik sağlıklı kontrol grubu dahil edildi. Hasta grubunun %60'ı (n=24) remisyon döneminde iken, %40'ı (n=16) ise atak dönemindeydi. Serum örneklerinde enzim bağlı immunosorbent yöntem (ELISA) yöntemi ile çalışılmak üzere HSP70'in aktif indüklenebilen formu olan HSPA1A ve yapısal formu olan HSPA8; HSP90 'ın aktif indüklenebilen formu olan HSP90AA2 ve yapısal formu olan HSP90AB1 belirlenmiştir. Çalışmanın sonraki basamağında ise MS atak dönemindeki hastalarda sağlıklı kontroller ile karşılaştırılarak kan örnekleri, HSP ilişkili genlerin ekspresyonlarının saptanması amacıyla laboratuvar ortamında gerçek zamanlı polimeraz zincir reaksiyonu (RT-PCR) yöntemiyle incelenmiştir. MS atak dönemindeki hastalarda serum HSP70 (p<0,001) ve HSP90 (p<0,001) seviyeleri remisyon ve sağlıklı kontrol grubuna göre anlamlı yüksek sonuçlanmıştır. Atak döneminde olan hastaların total kan RNA RT-PCR analizinde incelenen tüm genlerin ekspresyonları artış göstermiştir. Bu sonuçlar HSPA1A, HSPA8, HSP90AA2, HSP90AB1 serum ELISA düzeyleri ile paralellik göstermiştir. Total hasta ve kontrol grubu karşılaştırıldığında ise serum HSP90AB1 (p=0,045), HSP90AA2 (p=0,005), HSPA1A (p=0,018), HSPA8 (p=0,011) seviyeleri sağlıklı kontrol grubundan düşük saptanmıştır. Sonuç olarak; RRMS hastalarında atak döneminde, remisyondaki hastalar ve sağlıklı kontrollere göre HSP-70 ve HSP-90 seviyeleri anlamlı düzeyde yüksek bulunmuştur. Bu proteinlerin ekspresyonundan sorumlu genlerin de düzeyleri sağlıklı kontrollerden yüksek saptanarak, serum protein düzeyleri ile paralellik göstermiştir. Çalışmamızdan elde edilen bulgular ısı şok proteinlerinin stres ve inflamasyon ve nörodejenerasyonun hastalığın farklı evrelerinde farklı düzeyde patogeneze katkı bulunduğu, MS'de akut ve remisyon fazında gerek immünolojik gerek nöroprotektif özellikleri ile farklı davranış sergileyebileceğini düşündürmektedir.
dc.description.abstractMultiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS) characterized by inflammation and neurodegenerative processes. In MS, inflammation induces the synthesis and extracellular release of heat shock protein 70 (HSP70) and heat shock protein 90 (HSP90) within cells, which can trigger immune responses. The aim of this study is to determine the serum levels of HSP70 and HSP90 during the relapse phase, when inflammation is most agressive, and the remission phase, when acute inflammatory activity has decreased, in patients with Relapsing Remitting Multiple Sclerosis (RRMS). Additionally, the study aims to discuss the role of these proteins in the pathogenesis of the disease by analyzing their gene expression levels during both the relapse and remission phases. This prospective study included 40 patients aged 18-50 years (mean age: 33.55±10.04 years, 26 women) diagnosed with RRMS and followed up at the Neurology Multiple Sclerosis outpatient clinic of Göztepe Prof. Dr. Süleyman Yalçın City Hospital, along with an age- and sex-matched control group of 40 healthy individuals (mean age: 32.95±9.40 years, 26 women). Among the patient group, 60% (n=24) were in the remission phase, while 40% (n=16) were in the attack phase. In this study, the serum samples obtained were analyzed using the enzyme-linked immunosorbent assay (ELISA) method to measure the inducible form of HSP70, HSPA1A, and its structural form, HSPA8; as well as the inducible form of HSP90, HSP90AA2, and its structural form, HSP90AB1. In the next phase of the study, blood samples from patients in the MS attack phase were compared to healthy controls, and the expression of HSP-related genes was assessed in the laboratory using the real-time polymerase chain reaction (RT-PCR) method. In patients during the MS attack phase, serum levels of HSP70 (p<0.001) and HSP90 (p<0.001) were significantly higher compared to the remission and healthy control groups. The expression of all genes examined in the total blood RNA RT-PCR analysis of patients in the attack phase was increased. These results were consistent with the serum ELISA levels of HSPA1A, HSPA8, HSP90AA2, and HSP90AB1. When comparing the total patient and control groups, serum levels of HSP90AB1 (p=0.045), HSP90AA2 (p=0.005), HSPA1A (p=0.018), and HSPA8 (p=0.011) were found to be statistically lower than those in the healthy control group. In conclusion, during the attack phase in patients with RRMS, levels of HSP70 and HSP90 were found to be significantly higher compared to those in remission and healthy controls. The genes responsible for the expression of these proteins were also found to be elevated in comparison to healthy controls, showing consistency with serum protein levels. The findings from this study suggest that heat shock proteins contribute to the pathogenesis of stress, inflammation, and neurodegeneration at different levels during various phases of the disease. These proteins may exhibit distinct immunological and neuroprotective behaviors during the acute and remission phases of MS.
