dc.description.abstractAMAÇ. Pnömotoraks, akciğer ile göğüs duvarı arasında, plevral boşlukta, serbest hava toplanması ve akciğerin kollabe olmasıdır. Spontan pnömotoraks kendiliğinden gelişen bir tablodur ve toraks ile ilişkili en sık karşılaşılan cerrahi acil durumlardan biridir. Bu çalışmada amaçlanan acil tıp kliniğinde spontan pnömotoraks tanısı konmuş hastaların demografik özelliklerini değerlendirilmek ve bu hastalara yaklaşımımızı açıklamaktır. YÖNTEM. Bu çalışmada, İstanbul Medeniyet Üniversitesi Göztepe Eğitim Araştırma Hastanesi Acil Tıp Kliniği'nde 01.01.2013 ve 01.01.2015 tarihleri arasında spontan pnömotoraks tanısı konularak tedavisi ve takibi yapılan hastalar retrospektif olarak değerlendirildi. Hastaların dosyaları incelenerek yaş, cinsiyet, beden kitle indeksi, sigara öyküsü, eşlik eden hastalık öyküsü, geçirilmiş pnömotoraks öyküsü, başvuru şikayeti, pnömotoraksın lokalizasyonu ve boyutu, teşhisi, tedavi biçimi, cerrahi endikasyonları, nüks, morbidite ve mortalite oranları ile hastane yatış süreleri gözden geçirildi. İlk müdahale sonrasında hastaneye yatırılan olgularda cerrahi operasyon gereken olgular belirlendi. Tüp torakostomi işlemi yapılmış olan hastaların dosyaları incelenerek komplikasyon gelişen hastalarda komplikasyonun türleri ve oranları literatür bilgileri ile karşılaştırıldı. BULGULAR. SONUÇ. Spontan pnömotoraks acil tıp kliniği günlük pratiğinde sık karşılaşılan bir tablodur. Genellikle bu hastalarla ilk karşılaşan hekimler de acil tıp hekimleridir. Bu nedenle, bu tablonun yönetimi ve tedavi uygulamaları acil tıp hekimlerince iyi bilinmelidir. Bir tedavi yöntemi olarak tüp torakostomi, düşük komplikasyon oranlarıyla acil tıp kliniklerinde güvenle uygulanabilecek bir girişimdir.
dc.description.abstractOBJECTIVE. Pneumothorax consists in the collection of free air in the pleural cavity, between the chest wall and the lung, and a collapse of the latter. Spontaneous pneumothorax, a clinical situation developing with no apparent cause, is one of the most frequent emergencies in thoracic surgery. The objective of this study was to evaluate the demographic characteristics of patients diagnosed with spontaneous pneumothorax in an emergency medicine clinic and explain approach to these patients. METHODS. Patients diagnosed with a diagnosis of spontaneous pneumothorax treated and followed up from January 1, 2013 to January 1, 2015 at the Istanbul Medeniyet University Göztepe Teaching and Research Hospital Emergency Medicine Clinic were retrospectively reviewed. The patients' age, sex, body mass index, smoking history, concomitant disease history, previous pneumothorax history, presenting complaint, pneumothorax site and dimensions, diagnosis, treatment method, surgical indications, recurrence, morbidity, mortality and hospital stay durations were determined from the patients' charts. Patients who necessitated surgical procedures among those hospitalized after the initial intervention were determined. The types and rates of the complications noted to have been observed in patients who had undergone thoracostomy and tube insertion were compared to the available publications. RESULTS. Of the 50 patients with spontaneous pneumothorax evaluated in the study, a majority (82%) was male. Of these, 44.0% were smokers, with a statistically significant disparity between male and female patients (51.2% vs 11.1%, p=0.030). Looking at all the cases, 38 (76%) were primary and 12 (24%) secondary spontaneous pneumothorax. The pneumothorax occurred mainly during the winter season (46.0%). Patients with diagnosed comorbidities most frequently (74%) presented with dyspnea when coming to the emergency service. Patients without comorbidities most frequently exhibited chest pain (66.7%); such cases were determined to be mainly (86.1%) primary xii pneumothorax. Both these frequencies were statistically significant (p<0.01 and p=0.012, respectively). Of the patients with spontaneous pneumothorax, 17 (34%) were observed, while 33 (66.0%) underwent tube thoracostomy. Complications were encountered in 5 (14.7%) of the cases with thoracostomy drainage. When evaluating the hospitalization durations, the time to the end of tube drainage amounted to 4.889±2.201 days and the time to discharge to 5.000±2.243 days. CONCLUSION Spontaneous pneumothorax is a frequent situation in the daily practice of emergency medicine. The first physicians seen for these patients are generally emergency medicine doctors. Therefore, emergency medicine physicians should master the management and treatment of this condition. Tube thoracostomy is a safe intervention in emergency rooms, with a low complication rate.
