Laparoskopik apendektomi'de tek veya çift hem-o-lok kullanımının postoperatif komplikasyonlar üzerine etkisi: Randomize kontrollü çalışma

Abstract

AMAÇ. Non-komplike laparoskopik apendektomide apendiçekal güdük kapamasının tek veya çift hem-o-lok ile yapılmasının operasyon sonrası komplikasyonlar üzerine etkisini incelemek amaçlanmıştır. YÖNTEM. Bu prospektif randomize kontrollü çalışmamıza 1 Aralık 2017 – 1 Aralık 2020 tarihleri arasında kliniğimizde akut apendisit ön tanısıyla non-perfore laparoskopik apendektomi uygulanan vakalar dahil edildi. Hastalar randomize edilerek tek veya çift hem-o-lok uygulanmak üzere iki gruba ayrıldı. Hastaların demografik özellikleri, komorbiditeleri, operasyon bulguları, apendiks histopatolojik sonuçları kayıt altına alındı. Hastalar yedinci ve otuzuncu günde kontrole çağrıldı ve 6.ayda telefonla arandı. Takiplerini tamamlayan, kriteryayı karşılayan hastalar kohorta dahil edilerek gruplar arasında komplikasyon varlığı karşılaştırıldı. BULGULAR. Belirtilen süre zarfında başvuran 698 hastanın 447'si çalışmaya dahil edilerek tek hem-o-lok grubunda(Grup 1) 233 ve çift hem-o-lok grubunda(Grup 2) 214 hasta yer aldı. Gruplar arasında kadın/erkek, yaş, komorbidite varlığı ve komorbidite alt tipleri, beyaz küre, nötrofil sayısı, nötrofil Yüzdesi, CRP, T.Bil, İ.Bil ölçümleri benzer ve istatiksel anlamlı fark çıkmamıştır. Gruplar arasında operasyon öncesi radyolojik tanı alma oranı, peroperatif apendisit ciddiyet skoru, apendektomi materyallerinin histopatolojik sonuçları karşılaştırıldığında fark görülmemiştir. Altı aylık takip sonucunda 25 hastada apendektomiyle ilişkili komplikasyon görülmüştür. Grup 1 ve Grup 2'de sırasıyla komplikasyon oranı %5,5 ve %5,6 olup anlamlı fark bulunmamaktadır. SONUÇ. Non-komplike akut apendisit vakalarında laparoskopik güdük kapatılmasında tek hem-o-lok klip ile çift klip kullanılması benzer etkinlik ve güvenliğe sahip olup, tek klip uygulama kolaylığı ve maliyet açısından üstündür.

OBJECTIVE. It was aimed to examine the effect of closure of the appendiceal stump with single or double Hem-O-Lok on postoperative complications in non-complicated laparoscopic appendectomy. METHODS. Cases who underwent non-perforated laparoscopic appendectomy with a prediagnosis of acute appendicitis in our clinic between December 1, 2017 and December 1, 2020 were included in this prospective randomized controlled study. The patients were randomized and divided into two groups as single or double hem-o-lok. Demographic characteristics, comorbidities, operative findings and histopathological results of the patients were recorded. The patients were called for follow-up on the seventh and thirtieth day, and they were phone called at the sixth month. Patients who completed their follow-up and met the criteria were included in the cohort and the presence of complications was compared between the groups. RESULTS. During the specified period, 447 of 698 patients included in the study, 233 patients in the single hem-o-lok group (Group 1) and 214 patients in the double hem-o-lok group (Group 2). Male/female, age, comorbidity and comorbidity subtypes, white blood cells, neutrophil count, neutrophil percentage, CRP, T.Bil, I.Bil measurements were similar between the groups and there was no statistically significant difference. When the rate of preoperative radiological diagnosis, peroperative appendicitis severity score, and histopathological results of appendectomy materials were compared between the groups, no difference was observed. As a result of 6-month follow-up, complications related to appendectomy were observed in 25 patients. Complication rates in Group 1 and Group 2 were 5.5% and 5.6%, respectively, and there was no significant difference. CONCLUSION. The use of a single hem-o-lok clip and a double clip in laparoscopic stump closure in non-complicated acute appendicitis cases has similar efficacy and safety, and the single clip is superior in terms of ease of application and cost.
