Pandemi sonrasında obezite polikliniğinde takip edilen hastaların tedavi başarısı: Tek merkez deneyimi

Obezite, vücutta sağlığı olumsuz etkileyecek düzeyde aşırı yağ birikimiyle karakterize edilen kompleks bir kronik hastalık olarak tanımlanır. Son yıllarda, obezitenin dünya genelinde hızla artış gösterdiği ve önemli bir halk sağlığı sorunu haline geldiği gözlenmektedir. Bu çalışmada obezite polikliniğinde en az bir yıl takip edilen hastaların tedavi başarısını sosyodemografik değişkenler, takip durumu, obezite öyküsü, soy geçmişi, eşlik eden hastalıklar, ilaç kullanımı, menopoz durumu, sigara kullanımı, çocuk sayısı, yaşam tarzı ve laboratuvar parametreleri gibi çok sayıda faktörü dikkate alarak değerlendirmek amaçlanmaktadır. İstanbul Medeniyet Üniversitesi Göztepe Prof. Dr. Süleyman Yalçın Şehir Hastanesi Obezite Polikliniğinde 01.09.2021 ile 01.08.2023 tarihleri arasında takip edilmekte olan, dahil edilme ve dışlama kriterlerine uyan 312 hasta ile çalışma yürütülmüştür. Poliklinikte hasta takibinde kullanılan "obezite takip formu"ndan bu hastalara ait bilgiler toplanmıştır. Bu form sosyodemografik değişkenler, obezite öyküsü, eşlik eden hastalıklar, kullanılan ilaçlar, antipsikotik kullanımı, sigara kullanımı, doğum ve çocuk sayısı, menopoz durumları, egzersiz ve beslenme alışkanlıkları, soy geçmiş ve vizit notları gibi bilgileri içermektedir. Daha sonra TANİTA markalı vücut kompozisyon analizatörüyle (bioelektriksel impedans analiz ölçüm cihazı) hastaların başvuru esnasında ve son vizitteki kilo, boy, VKİ, yağ, su ve kas ölçümleri ilk ölçüm ve son ölçüm değeri olarak kaydedilmiştir. Ayrıca hastane bilgi sisteminden hastalara ait glukoz, HbA1c, insülin, c-peptid, ürik asit, AST, ALT, total kolesterol, LDL, HDL, trigliserid ve CRP kan parametrelerinin ilk ve son değerleri kaydedilmiştir. Çalışma tek merkezli ve retrospektiftir. İstatistiksel analizler SPSS (Statistical Package for the Social Sciences) versiyon 23.0 kullanılarak yapılmıştır ve p değerinin <0.05 olduğu durumlar istatistiksel olarak anlamlı kabul edilmiştir. Çalışmaya katılan hastaların %44,2'si 30-50 yaş aralığında olup, bu yaş grubu en yüksek oranı temsil etmektedir. Cinsiyete göre, hastaların %88,5'i kadın ve %11,5'i erkektir. Medeni durumlarına göre, hastaların %80,1'i evli, %11,7'si bekar ve %8,2'si boşanmış veya dul olarak saptanmıştır. Eğitim düzeyine ilişkin verilerde, en yüksek oran %36,8 ile ilkokul mezunlarına ait olup, bunu %30,0 ile lise mezunları ve %23,0 ile üniversite mezunları izlemektedir. Meslek dağılımında, en yüksek oran %54,3 ile ev hanımlarına, ardından %15,1 ile mavi yakalı çalışanlara, %13,3 ile emeklilere ve %11,2 ile beyaz yakalı çalışanlara aittir. Çalışmamız yaşam tarzı müdahalesiyle hastaların %51,3'ünün %5 ve üzerinde kilo verdiğini, kilo kaybına bağlı olarak metabolik ve biyokimyasal göstergelerde önemli iyileşmeler sağladığını ortaya koymuştur. Ayrıca, kilo verme sürecinde düzenli poliklinik takibi, doğru beslenme ve egzersiz gibi sağlıklı yaşam alışkanlıklarının, tedavi başarısında kritik bir rol oynadığı tespit edilmiştir.
Obesity is defined as a complex chronic disease characterized by excessive fat accumulation in the body, negatively affecting health. In recent years, it has been observed that obesity has been rapidly increasing worldwide, becoming a significant public health issue. This study aims to evaluate the treatment success of patients who have been followed for at least one year in the obesity clinic, taking into account various factors such as sociodemographic variables, follow-up status, obesity history, family background, comorbidities, medication use, menopause status, smoking, number of children, lifestyle, and laboratory parameters. The study was conducted with 312 patients who met the inclusion and exclusion criteria and were being followed between 01.09.2021 and 01.08.2023 at the Obesity Clinic of Istanbul Medeniyet University Göztepe Prof. Dr. Süleyman Yalçın City Hospital. The information of these patients was collected from the "obesity follow-up form" used in patient follow-up at the clinic. This form includes details such as sociodemographic variables, obesity history, comorbidities, medications used, antipsychotic use, smoking status, birth and number of children, menopause status, exercise and nutritional habits, family history, and visit notes. Additionally, the body composition analyzer branded TANITA (bioelectrical impedance analysis measurement device) recorded the weight, height, BMI, fat, water, and muscle measurements of the patients during their initial visit and final visit as the initial and final measurement values. The initial and final values of glucose, HbA1c, insulin, c-peptide, uric acid, AST, ALT, total cholesterol, LDL, HDL, triglycerides, and CRP blood parameters were also recorded from the hospital information system. The study is single-centered and retrospective. Statistical analyses were performed using SPSS (Statistical Package for the Social Sciences) version 23.0, and cases where the p-value was <0.05 were considered statistically significant. Among the patients participating in the study, 44.2% were in the 30-50 age range, representing the highest proportion. By gender, 88.5% of the patients were female, and 11.5% were male. Regarding marital status, 80.1% of the patients were married, 11.7% were single, and 8.2% were divorced or widowed. In terms of educational level, the highest proportion was among primary school graduates at 36.8%, followed by high school graduates at 30.0%, and university graduates at 23.0%. Regarding occupation, housewives made up the highest proportion at 54.3%, followed by blue-collar workers at 15.1%, retirees at 13.3%, and white-collar workers at 11.2%. Our study revealed that 51.3% of the patients lost 5% or more of their body weight through lifestyle intervention, and significant improvements in metabolic and biochemical indicators were achieved due to weight loss. Furthermore, it was found that regular clinic follow-up, healthy lifestyle habits such as proper nutrition and exercise played a critical role in treatment success.
